Job Description:
The Memory Support Program Manager coordinates the memory
support program to ensure that the physical, social & emotional
needs of the residents and their families are met. As a
representative and team member of the company, this position is
expected to present oneself in a manner that reflects
professionalism and ensures resident satisfaction. This is a
full-time position | Schedule: Monday - Friday. Weekend as needed.
Pay range is $70,500K- $102,000K a year depending on experience. To
be successful in the role, you would have: Must have at least 3 to
5 years prior related experience with people living with dementia
and at least 1 year of supervision experience. High School Diploma
or equivalent; Bachelor Degree preferred Prefer 2 years’ experience
in assisted living management. First Aid certified and CPR training
required Work Duties Keeps the HumanGood Memory Support Program
Philosophy at the core of all planning and implementation of
activities and care to meet the specific needs of residents;
Supervises Team Members in the Memory Support; supervision includes
management of work schedules, hiring, training, evaluating,
employee development, coaching, discipline, salary increases, and
adjustments; Leads by example and collaborates with Director of
Community Life to ensure team members engage residents in programs
Collaborates with Clinical partners to coordinate resident moves
and transfers within the Memory Support; Assure that resident
profile is completed (prior to move-in preferable; no later than 3
business days after move-in) and shared with team members; Provides
orientation for new residents and family members; Conduct and
promote support and/ or educational groups for residents/ families;
Communicates with resident family members in a respectful and
caring manner and provides information regarding residents’ daily
life as appropriate; uses tactful, diplomatic communication
techniques in potentially sensitive or emotionally charged
situations; Conducts Memory Support team member meetings, makes
recommendations for training and communications and system
improvements; Coordinates with Director of Assisted Living when
significant changes of residents occur which may lead to level of
care change or adjustment; Prepares and updates Level of Care
Transfer Notice and Service Plans for Memory Support residents in
collaboration with the Director of Wellness and Assisted Living;
Monitors and focuses on non-pharmacological solutions of behavioral
expressions; involves other team members, leaders, and CSC members
as needed; coaches team members to provide behavioral
interventions; Collaborates with Dining Team Members to ensure
dignified dining experience Maintains readiness for licensing and
accreditation survey; Participates in Safely You Huddle Calls,
reviews falls and responses to falls; ensures permission for Safely
You is obtained from families (in agreement) and maintains
documentation of permission; Ensures Wandergard system (when in
place) functions per guidelines; Leads and/ or participates in
resident related meetings (Resident Review Committee Meetings, Care
Plan Meetings, Family Meetings, etc.) Completes required state
training programs for Memory Support; Manage department budget
including census targets and purchases supplies as needed Attends
in-service training workshops and meetings as required; Conducts
work tasks safely and in compliance with the community safety
program; Provides effective and courteous service to all residents,
guests and team members; Promotes and protects the rights of
residents; Responds to emergencies in a prompt, calm manner;
Performs monthly safety audits and submits work orders as needed;
and Performs various related essential duties as required. What's
