Physical Therapist Location: Rancho Cordova, CA Agency: Cynet Health Pay: $2,283 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/31/2026 About the Position Job Title: Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum 2 years License: Current Physical Therapy License required Certifications: BLS Certification, NPI required Must-Have: Home Health experience including OASIS required. EMR HomeCare Home Base (HCHB) experience preferred. Current driver's license and proof of automobile insurance required. Description: Provide physical therapy services to patients/clients according to a written physician's plan of care. Assess, evaluate, and update therapeutic/rehabilitative/functional status. Participate in the development of the total plan of care. Evaluate home environment and assess for muscle strength, mobility, gait, and range of motion. Direct physical therapy treatment and instruct patients/clients and families/caregivers in the use and care of therapeutic appliances. Determine priority needs for physical therapy. Report the patient's/client's reaction to treatment or changes in condition to the physician. Initiate physical therapy program and instruct other personnel and/or family members in specific phases of physical therapy. Participate in case conferences. Train patients/clients in the use of prosthetic devices as needed. Supervise homemaker/Home Health Aides when they are participating in the rehabilitation regime. Identify patient/client and family needs for other home health services and refer as necessary. Prepare and submit clinical and progress summaries based on the attainment of goals. Observe, record, and report information on the patient's condition to the attending physician and in the patient's health record. Provide physical therapy consultation to families/caregivers when indicated. Provide in-service education programs for nursing agency personnel as needed. Participate in peer consultation processes and ongoing staff competency assessment. Comply with and understand relevant regulations and licensure requirements as they pertain to the position.
