for a Care Manager/Care Coach in San Francisco. Details: Full-time
40 hours Include remote work, Health Center or Home Care visits
DUTIES / RESPONSIBILITIES: Work closely with participants,
caregivers, providers to perform day-to-day activities with
participants in the community, at healthcare provider's offices and
in participant's homes. Serve as patient advocate and program
liaison. Quickly develop relationships inside and outside of the
organization Expected to be a role model for healthy
behavior/lifestyle choices and be able to operate with minimal
supervision. QUALIFICATIONS: Graduate of an accredited college with
nursing, social work or gerontology degree. Experience working with
older adults Experience in advocating for the health and well-being
of older adults; empathetic Familiarity with social service, health
and community agencies in San Francisco Knowledge and understanding
of HIPAA regulations Experience with motivational interviewing and
active listening skills Experience in facilitating positive
